Health Paper Request
Use the link below to submit information for a health paper. If an appointment is needed to inspect the animal(s) please call the office at 815-369-2400 to schedule. Inspections are needed if a doctor has not seen the animal(s) within 10 days of writing the paper. Papers are good for 30 days if going out of state, and 90 days if staying within the state of Illinois.
