Health Paper Request

You may also request a health paper form from the office via email: [email protected]. If an inspection of the animal(s) is needed, please call 815-369-2400 to schedule with a doctor. An inspection is needed within 10 days of writing the paper. Health papers for out of state movement are good for 30 days. Papers written for animals staying within the state of Illinois are good for 90 days.
